Physician-Led Diagnostic Discernment

Diagnostic Consultation

A Consultation Built Around Your Whole Story, Not Just Your Next Fifteen Minutes

The Diagnostic Consultation is a structured, physician-led experience, before, during, and after a 60-minute consultation with Dr. Wanda Simmons Clemmons, MD, designed to bring order to a health story that may currently feel scattered across records, providers, and appointments.

The Journey

A structured consultation begins by making room for the full history, not simply the latest appointment.

Why This Is Different

Access to Your Records Was Never the Real Problem

You may already have your medical information. What's often missing isn't access, it's knowing what matters inside it. Recognizing which patterns are worth naming. Understanding what you've actually been told, not just what was documented. Knowing what question comes next.

This is what separates the Diagnostic Consultation from a records portal, a binder, or another place to store what you already have. This is a structured process for making sense of it.

Phase One
Your Story, Prepared With Intention
  1. At least three days before your consultation:
  1. 1. You submit your medical records. Dr. Wanda spends approximately an hour reviewing them directly, not delegated to intake staff.
  2. 2. You complete structured pre-consultation materials designed to surface what matters, not just what's easiest to describe.
  3. 3. You identify The Three Things™, the concerns you most want clarity on, and separately, The Rest of the Story, everything else worth knowing, even if it doesn't feel urgent enough to lead with.
Phase Two
Sixty Minutes, Physician-Led Throughout

Your three priorities guide the consultation, but they don't constrain it. Dr. Wanda reviews your complete history, records, and stated concerns, and exercises clinical judgment about what actually requires priority, which may confirm what you already suspected, or may surface something you hadn't named as a priority at all.

Your priority and your clinical priority aren't always the same thing, and that's the point of having a physician in the room.

If additional procedures, such as cognitive testing, are appropriate during your visit, they'll be identified and documented separately.

Phase Three
You Leave With More Than Notes

Every consultation concludes with a Personalized Diagnostic Consultation Report, built on a structured reasoning sequence: History → Fracture → Pattern → Clinical Priorities → Next Questions. A one-page clinical summary may accompany the full report, something you can bring directly to your primary physician.

What accompanies your report

  • Physical Health Stewardship resources from the collection below
  • 30 days of complimentary MetAlign™ Silver access
  • A signed copy of From Deception and Deceit to Destiny
  • Additional selected resources, delivered as a complete physical portfolio, not a folder of loose paperwork

This consultation is not a promise that every medical question will be resolved. It's a structured, physician-led process for leaving with more clarity and a clearer next step than you arrived with.

What comes with every Consultation

The Medical Paraclete™ Health Stewardship Collection

Five physician-designed tools, each addressing a different piece of what it actually takes to participate in your own healthcare, not just hold onto your own records.

01

Your Medical Story, Organized™

Know the story.

Not a system for storing records, a way to think through what you believe has happened, what physicians have told you, what you actually understand, and where it's still unclear.

02

Patterns & Signals™

Notice what repeats.

Builds observational literacy without encouraging self-diagnosis. The goal isn't “therefore, I have X,” it's “this may be worth asking about.”

03

Three Things™

Name what matters now.

The same visit-preparation approach used inside the consultation itself, built as a reusable tool for every future physician visit you'll ever have.

04

Did I Hear You Right?™

Confirm what you understand.

Rather than asking “do you understand,” this resource helps you say back what you heard, so any gap in understanding gets caught and corrected in the room, not after.

05

What Happens Next?™

Know the next step.

Clarifies what happens next, who's responsible for it, when it should happen, and what to do if things don't proceed as expected.

Your medical chart tells you what was documented. This collection helps you understand what you actually know, and what to do with it.

Scope, Named Clearly

 What This Is, and Isn't

The Diagnostic Consultation and the Health Stewardship Collection do not interpret your medical record, diagnose your symptoms, or replace your existing healthcare relationships. Their function is physician-designed health stewardship, helping you understand your story, notice what's worth naming, prepare what matters, confirm what you heard, and leave knowing what happens next. Availability is currently limited by physician licensure and varies by state.

Bring Clarity to the Whole Picture

Ready to Begin...

The patient brings the story. The physician discerns what requires clinical priority. You leave understanding what comes next.

YOUR HEALTH IS WITHIN REACH.
